ABSTRACT

Objective:To evaluate neutrophil/lymphocyte ratio(NLR) and the model for end-stage liver disease-sodium(MELD-Na)score in predicting short-term prognosis of patients with HBV-related acute-on-chronic liver failure(HBV-ACLF).Methods:A total of 234 consecutive HBV-ACLF patients(194 males and 40 females, aged 23-85 years)admitted to Hangzhou Xixi Hospital from January 2019 to December 2021 were enrolled. According to the 12-week clinical outcomes, patients were divided into good prognosis group( n=141)and poor prognosis group( n=93). Univariate and multivariate Logistic regression were performed to identify independent risk factors for poor prognosis of HBV-ACLF patients. Receiver operating characteristics(ROC)curve was applied to evaluate the accuracy of risk factors in predicting short-term prognosis of HBV-ACLF patients. Results:The age [(48.7±11.9) vs. (52.5±9.9) years old, t=-2.59, P=0.011], proportion of males [78.0%(110/141) vs. 90.3%(84/93), χ2=5.99, P=0.014], total bilirubin[202.9(141.2, 287.6) vs. 320.0(224.4, 400.0) μmol/L, Z=-5.14, P<0.001], creatinine [71.0(59.0, 78.0) vs. 81.0(64.0, 111.0)μmol/L, Z=-3.98, P<0.001], international normalized ratio[1.66(1.52, 1.86) vs. 1.91(1.66, 2.27), Z=-5.46, P<0.001], leukocyte count[5.16(3.99, 6.95)×10 9/L vs. 6.57(4.83, 8.30)×10 9/L, Z=-4.14, P=0.001], NLR[2.77(2.02, 3.55) vs. 5.48(3.44, 8.53), Z=-8.48, P<0.001], MELD score[22.0(20.0, 24.0) vs. 26.0(24.0, 29.0), Z=-9.22, P<0.001], MELD-Na score[22.8(20.0, 25.6) vs. 29.0(25.0, 36.0), Z=-9.16, P<0.001], liver cirrhosis[77.3%(109/141) vs. 88.2%(82/93), χ2=4.41, P=0.036], hepatorenal syndrome[4/141(2.8%) vs. 12/93(12.9%), χ2=8.91, P=0.003] and the proportion of artificial liver treatment[21/141(14.9%) vs. 24/93(25.8%), χ2=4.30, P=0.038] were significantly elevated in poor prognosis group compared with survival group. Logistic regression analysis showed that NLR( OR=3.76, 95 %CI: 2.10-6.74, P<0.001)and MELD-Na score( OR=2.24, 95 %CI: 1.17-4.29, P=0.015) were independent risk factors for poor short-term prognosis of HBV-ACLF patients. The area under the ROC curve(AUC)of NLR, and MELD-Na for the short-term prognosis of HBV-ACLF patients was 0.792 and 0.822, respectively. The AUC of the combination of NLR with MELD-Na was 0.858, which was significantly higher than that of NLR( Z=-3.04, P=0.001) or MELD-Na score( Z=-2.16, P=0.031)alone. Based on the cut-off value of the combined model, patients were classified into high combined model score (≥0.04) group and low combined model score (<0.04) group, the survival rate of the high group was significantly higher than that of the low group( χ2=67.47, P<0.001). Conclusions:NLR and MELD-Na score are independent risk factors of the short-term prognosis of HBV-ACLF patients. The combination of NLR and MELD-Na score will be beneficial to predict the short-term prognosis of HBV-ACLF patients.

ABSTRACT

OBJECTIVE: To analyze the epidemiological characteristics of pneumoconiosis among migrant workers without liability subject(hereinafter referred to as Pneumoconiosis without Liability Subject) in Hunan Province. METHODS: The cases of pneumoconiosis without liability subject from 2017 to 2019 in Hunan Province were selected as the research subjects using typical sampling method. They were clinical diagnosis by occupational disease diagnostic institutions. The distributions of age, gender, length of service, area, type of work, type of pneumoconiosis, pneumoconiosis stage and the situation of poor households with filing and registration card were analyzed. RESULTS: A total of 18 870 cases of pneumoconiosis without liability subject were clinically diagnosed in Hunan Province from 2017 to 2019. The patients were mainly males(accounting for 99.8%), with the age ranged 50-65 years old(64.7%). Most of them had dust exposure service length of 5-29 years(78.4%). The cases of stage Ⅰ, Ⅱ and Ⅲ pneumoconiosis accounted for 32.2%, 26.0% and 41.8% respectively. The main types of disease were coal workers′ pneumoconiosis and silicosis(accounted for 99.3%). The first five geographical distributions were Chenzhou City, Zhuzhou City, Hengyang City, Yiyang City and Shaoyang City, accounting for 17.9%, 14.6%, 14.1%, 11.8% and 9.2% respectively. The distribution of work types were mainly mine-related jobs(91.3%). There were 1 774 cases who had complications(9.4%), of which the top three complications were emphysema, pulmonary and bronchial infection and tuberculosis. There were 3 662 cases with poor households archives and cards(19.4%). CONCLUSION: The hazards of pneumoconiosis among migrant workers in Hunan Province should not be ignored. In 2017, Hunan Province took the lead in launching a large-scale basic medical treatment and rescue operation for migrant workers with pneumoconiosis, which helped solve the problem of pneumoconiosis in migrant workers who had no professional history certification and responsible employer.

ABSTRACT

Objective:To explore the effect of task-based rehabilitative training on neural circuit plasticity and forelimb motor function after C5 spinal cord injury in mice. Methods:A total of 21 healthy C57/BL mice were randomly and equally divided into sham group, model group and training group. The model was established by left C5 spinal cord crush injury. The lamina was removed without damaging the spinal cord in the sham group. Four weeks after injury, the training group received task-based rehabilitative training for four weeks. The horizontal ladder and rearing tests were used to assess motor function for forelimb before injury, and three days, two weeks, four weeks, six weeks and eight weeks after injury. The axons of the corticospinal tract in all mice were observed six weeks after injury by using biotinylated dextran amin (BDA) anterograde tracing. Eight weeks after injury, motor-evoked potential was applied to measure nerve conduction velocities in forelimb, while the axon sprouting and syntagmatic relation of neuron in the anterior horn of gray matter above lesion were observed by immunofluorescence double-labeling of BDA/neuron-specific nuclei protein (NeuN); the expression of Synapsin in the anterior horn of gray matter at lesion was observed by immunofluorescence double-labeling of NeuN/Synapsin I. Results:Eight weeks after injury, the latency of P1 and N1 was longer in the model group than in the sham group (P < 0.05), and was shorter in the training group than in the model group (P < 0.05). Compared with the sham group, the error rate of left forelimb increased, and the usage rate decreased (P < 0.05) in the model group and the training group; compared with the model group, the error rate of left forelimb decreased six weeks and eight weeks after injury (P < 0.05), and the usage rate increased eight weeks after injury (P < 0.05) in the treatment group. Compared with the model group, more axon sprouting co-localized with neurons in the anterior horn of gray matter above lesion (P < 0.05), and the expression of Synapsin I increased in the training group (P < 0.05). Conclusion:Task-based rehabilitative training could promote the neural circuit plasticity and improve the motor function of forelimb after spinal cord injury in mice.

ABSTRACT

Rehabilitation is an essential treatment for knee osteoarthritis. Rehabilitation evaluation is the foundation of rehabilitation treatment. The items of rehabilitation evaluation for knee osteoarthritis include pain, proprioception, range of motion, muscle force, balance, gait, quality of life, and so on. When evaluating the patient's pain, researchers should not only focus on Visual Analogue Score (VAS) outcomes, but also consider the patient's body function and activity level, and whether there is neuropathic pain. Furthermore, isokinetic dynamometer can not only be used for quantitative assessment for muscle strength, but also for the quantitative evaluation for patient's proprioception. Moreover, Community Balance and Mobility Scale has no ceiling and floor effects which also is a reliable and useful tool for assessing the balance function in patients with knee osteoarthritis, compared with Berg Balance Scale. The evaluation for body structure in patients with knee osteoarthritis mainly includes the radiological examination and physical examination. There are a lot of scales to evaluate the quality of life in patients with knee osteoarthritis, and Osteoarthritis of Knee and Hip Quality of Life fits most. Further researches and promotion of evaluation tools for knee osteoarthritis are needed in the future.

ABSTRACT

<p><b>OBJECTIVE</b>To review and investigate the optimal preoperative diagnostic means and treatment principles of hepatic angiomyolipoma (HAML).</p><p><b>METHODS</b>The clinical features, treatment, prognostic and follow-up data of 169 HAML patients treated between January 1992 and May 2010 were retrospectively analyzed. The median age of the patients, including 46 male and 123 female (male/female, 1/2.7), was 45 years (range, 17 - 73 years). The mean case history was 0.54 year with a range of 2 d to 16 years.</p><p><b>RESULTS</b>Among the 169 patients, 149 patients (88.2%) had a solitary tumor and 96 patients (56.8%) were detected in the right lobe. The overall preoperative diagnostic rate was 13.6% and 119 patients (70.4%) were misdiagnosed as hepatocellular carcinoma or hepatic cavernous hemangioma. The diagnostic accuracy of MRI is higher than CT in distinguishing the nature of the tumor (χ² = 5.508, P = 0.019). One hundred and sixty-eight patients received surgical resection and one received percutaneous microwave coagulation therapy. One patient occurred postoperative hemorrhage and 3 patients developed hydrothorax. The postoperative mortality and recurrence for all the patients were 0. Postoperative pathology confirmed the diagnosis of hepatic angiomyolipoma. Follow-up study showed a benign course and no signs of recurrence.</p><p><b>CONCLUSIONS</b>MRI is the main diagnostic means of HAML. Treatment strategies of HAML depends largely on tumor size, location and growth rate. Surgical management is suggested to patients with the following criteria: (1) tumor size greater than 5 cm; (2) with clinical symptoms; (3) faster tumor growth; (4) the tumor located at 1, 4, 5, 8 segments of liver.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy of percutaneous laser ablation (LA) in the treatment for portal vein tumor thrombus (PVTT) of hepatocellular carcinoma (HCC).</p><p><b>METHODS</b>The PVTT of HCC patients were treated through percutaneous transhepatic laser ablation (PTLA). The survival rate, thrombus size, blood flow of embolized portal vein by thrombus, liver function, ascites and clinical presentation were observed.</p><p><b>RESULTS</b>The 6-month, 1-year and 2-year survival rate of these 93 patients were 82.8%, 53.0% and 34.1%, respectively. In 11 patients with partially occluded portal vein by PVTT, the cut-surface of the PVTT diminished significantly 6 months after LA. The color blood stream signal was seen again one day after LA in all of the other 82 patients with totally occluded portal vein by thrombus, and it could still be seen in 67 of those one month later, 57 (of 71) 3 months later, 40 (of 57) 6 months later, 27 (of 32) 1 year and 4 (of 6) 2 years later after LA. In the 38 patients who survived over 1 year, PVTT was gradually atrophied and disappeared eventually in 14, PVTT was atrophied and the portal vein changed into honeycomb-like appearance in 14. In the remaining 10 patients, PVTT continued to grow and made the portal vein enlarged. It was also observed that liver function, clinical symptom and ascites were improved in various degree after LA.</p><p><b>CONCLUSION</b>Percutaneous laser ablation might be an effective and safe treatment method for controlling portal vein tumor thrombus of hepatocellular carcinoma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To introduce a newly developed procedure in the control of portal vein tumor thrombus (PVTT) of hepatocellular carcinoma (HCC), and evaluate the efficacy and indicate of this method.</p><p><b>METHODS</b>The PVTT of HCC patients were treated by percutaneous transhepatic laser ablation (LA). The blood flow of PVTT embolized portal vein, live function, ascites and clinical presentation was observed.</p><p><b>RESULTS</b>Twenty-four HCC patients, with a total of 30 PVTT portal vein and its main branch were treated with LA. There were no any blood flow signal in Doppler color Ultrasonography in these PVTT embolized portal vein before treatment. After treatment, blood flow was reappearance in all cases within one week. The continued patency blood flow was observed in 16 portal vein and continued but not patency blood flow in other 12 portal vein within 30 days. The continued patency blood flow was observed in 18 portal vein within 90 days. The improvement of liver function and clinical symptom. The reduction of ascites was observed in varying degrees.</p><p><b>CONCLUSION</b>LA treatment might be a effective and safe procedure in the control of portal vein tumor thrombus of HCC.</p>

ABSTRACT

Objective To investigate the effects of low frequency ultrasound (LFU) on the proliferation and apoptosis of smooth muscle cells (SMCs) in rabbits with carotid atherosclerosis(CA).Methods CA models were established in 30 New Zealand rabbits using a high fat diet and air-drying.They were randomly divided into a control group and four LFU groups:group A received 0.5 W/cm~2 LFU for 5 min/d,group B 0.5 W/cm~2 for 10 min/d, group C 1 W/cm~2,5 min/d,and group D 1 W/cm~2,10 min/d.The rabbits' carotid arteries were autopsied after 20 d of the LFU treatment.The expression of PCNA and TUNEL staining were used to explore the proliferation and apopto- sis of SMCs,and the proliferation rates (PRs) and apoptosis rates (ARs) were calculated.Results Compared with the control group,the PRs in groups B,C and D were significantly lower,while the ARs in groups B,C and D were significantly higher.There was no significant difference in ARs or PRs among groups B,C and D.Con- clusion LFU can induce SMC apoptosis and inhibit SMC proliferation in rabbits with CA.

ABSTRACT

Objective To evaluate the therapeutic effect of rapid angulation rotation traction for lumbar intervertebral disc protrusion. Methods A total of 88 patients (66 male and 22 female;age: 18-65), with a history of 2 days to 10 years were analyzed. Among them 20 cases were central protrusion 68 cases were lateral protrusion. Traction range: 60-65 mm, angle: 20°-25°, rotation degree: 20°-25°, traction time: 3 s-1 min. The process of treatment was computer designed. The traction was then 1 to 3 times, with a interval of 4-7 days between two treatments. Results The effective rate was 96% for the case with history <1 month 96% and 85% for cases with history >1 month (P<0.01). The effective rate was 62% for central protrusion, and 87% for lateral protrusion respectively. χ2 test showed the difference was not significant. Conclusion The rapid angulation rotation is an effective treatment for lumbar inter-vertebral disc protrusion and this non operative method should be used as early as possible.
